• Reserves Request form
  • Request either electronic or traditional Reserves. If you already have a form on file for your course, you only need to fill out the second page of this form when requesting other materials to be added to the existing course.
• Video Booking Request form
  • Advanced booking of videos/DVD's for classroom presentation. Twenty four (24) hours advanced notice is required.
• Video/DVD Request for Student Presentation form
  • Request videos that are required for student presentations in class. This form requires faculty signature and ID number.
• Student Pickup for Faculty Video/DVD Request form
  • Authorizes a student to pickup videos/DVD's for faculty member.
